Job Title: Care Coordinator

We are seeking a highly motivated and compassionate Care Coordinator to join our team at Wellpoint Care Network. As a Care Coordinator, you will play a critical role in supporting families and youth in our community by providing community-based family support for mental health and behavioral needs.

Job Summary:

The Care Coordinator will work closely with families, youth, and community resources to provide a strengths-based approach to support and healing. This role requires a strong understanding of trauma-informed care, cultural humility, and the ability to work effectively with diverse populations.

Key Responsibilities:
  • Perform clinical assessments, including intake interviews and screens for new referrals using a trauma-informed and recovery-oriented perspective.
  • Develop, direct, and implement client treatment plans, interventions, skill training, and crisis management.
  • Set appropriate expectations for family involvement upon admission.
  • Demonstrate family investment and participation in treatment planning and ongoing treatment.
  • Develop a network of community support and resources and connect clients with these resources as appropriate.
  • Demonstrate relationship building and information sharing supporting a collaborative system of care.
  • Establish and maintain working knowledge of the WCHSD programs, resources, and supports.
  • Complete all necessary paperwork timely and utilize trauma-informed, strength-based language.
Requirements:
  • Bachelor's Degree in Social Work (BSW) required.
  • Certification as a Social Worker (CSW) preferred.
  • Master's Degree in Social Work or related field preferred.
  • Ability to work in-home with children and families, as well as coordinate services with ancillary providers within the community.
  • Ability to multi-task and prioritize work within the community; in conjunction with timely completion of documentation to support the work completed in the community.
  • Excellent written and oral communication skills.
  • Demonstrates ability to work independently and effectively with staff, clients, caregivers, and purchasers.
  • Must be honest, dependable, and able to meet deadlines; Self-motivated and able to work independently.
  • Valid driver's license with an acceptable driving record, reliable transportation, and meet agency auto insurance requirements.
  • Required or reliable transportation to conduct on-site learning throughout the state.
  • Ability to demonstrate critical thinking skills, detail-oriented, highly effective verbal, and written communication skills.
  • Demonstrate an ability to effectively understand and practice cultural humility and ability to work and communicate respectfully with individuals from diverse backgrounds.
  • Proficient in the usage of computer software; demonstrate an understanding of computer file systems and databases, e.g., Microsoft Office, Excel, Outlook, Teams, ADP, etc.

  • Public Service Loan Forgiveness (PSLF) - Wellpoint employees may be eligible for loan forgiveness under the Public Service Loan Forgiveness program.
  • Interaction with personnel, clients, family members, visitors, volunteers, interns, and vendors under all conditions and circumstances.
  • Physical exam, drug screen, motor vehicle report, and background checks are required for this position.
