Job Title: Care Coordinator

Wisconsin Community Services (WCS) is seeking a dedicated and compassionate Care Coordinator to join our Comprehensive Community Services (CCS) program. As a Care Coordinator, you will play a vital role in helping individuals understand, navigate, and grow through the CCS program.

Job Summary:

The Care Coordinator will work closely with individuals, service providers, and other team members to ensure seamless coordination of services and support. This role requires strong assessment and interviewing skills, as well as the ability to maintain and promote positive working relationships within WCS and with external partners.

Key Responsibilities:
  • Establish individuals' eligibility for CCS services using a Mental Health/AODA Functional Screen.
  • Assist individuals in identifying services from the CCS service array that address their recovery needs.
  • Broker services as needed with other providers in the CCS network.
  • Work with individuals to build a recovery support team of professionals and natural supports.
  • Develop and implement individualized recovery treatment plans.
  • Facilitate recovery meetings with individuals and their teams.
  • Monitor progress and ensure quality care and goal achievement.
  • Document all contact with individuals and team members.
Requirements:
  • Bachelor's degree in Social Work or a related field required; Master's degree preferred.
  • Two years of experience working in the Mental Health field required.
  • Experience working with individuals with substance use needs preferred.
  • Meet WCS requirements for criminal and caregiver background checks and driver's record.
  • Valid driver's license, automobile, and sufficient insurance required.
Working Conditions:

This role requires a mobile and flexible work style, with occasional transportation of clients, home visits, and attendance at meetings. A driver's license and vehicle are required. The position is based in Milwaukee County, with occasional travel to other locations.
