Job Title: CCS Adult Care Coordinator

We are seeking a highly skilled and compassionate Care Coordinator to join our dynamic team at JFS, Inc. in the greater Milwaukee area. As a Care Coordinator, you will play a vital role in providing rehabilitative and recovery services to individuals enrolled in the Comprehensive Community Services (CCS) program.

About the Role:

As a Care Coordinator, you will work closely with consumers to identify recovery goals, develop customized recovery plans, and provide support to improve or maintain their quality of life, independence, health, and overall wellness. You will work collaboratively with a dedicated team of providers to ensure that consumers receive the necessary programs and supports to achieve their recovery goals.

Responsibilities:
  • Conduct assessments of consumers to identify their needs and develop individualized recovery plans.
  • Provide case management services to consumers, including monitoring and supporting their progress towards their recovery goals.
  • Collaborate with a multidisciplinary team of providers to ensure that consumers receive comprehensive and coordinated care.
  • Develop and maintain relationships with community resources and providers to ensure that consumers have access to necessary services and supports.
  • Document consumer progress and maintain accurate records of services provided.
Requirements:
  • Bachelor's or Master's degree in a Human Services field (e.g. Social Welfare, Psychology, etc.).
  • At least one year of paid formal experience working with adults providing case management or related services.
  • Experience in behavioral health, mental health, and/or addiction is preferred.
  • Excellent interpersonal, organizational, critical thinking, problem-solving, and independent thinking skills.
  • Ability to work independently with minimal supervision.
  • Excellent verbal and written communication skills.
  • Ability to maintain confidentiality and respect consumer privacy.
