Job Title: CCS Adult Care Coordinator

We are seeking a highly skilled and compassionate Care Coordinator to join our dynamic team at JFS, Inc. Our Care Coordinators play a vital role in providing care coordination services to individuals enrolled in the Comprehensive Community Services (CCS) program in the greater Milwaukee area.

About the Role:

As a Care Coordinator, you will work closely with individuals to identify recovery goals, develop customized recovery plans, and provide support to help them achieve their goals. You will work with a dedicated team of providers to decide which programs and supports will assist the individual in reaching their recovery goals.

Responsibilities:
  • Conduct assessments of, monitor, and support CCS consumers with a professional and caring methodology.
  • Meet with individuals in their homes or out in the community to identify recovery goals and develop customized recovery plans.
  • Work with a dedicated team of providers to decide which programs and supports will assist the individual in reaching their recovery goals.
  • Provide support to help individuals achieve their recovery goals.
Requirements:
  • A 4-year Bachelor's or a Master's degree in a Human Services field (Social Welfare, Psychology, Social Work, etc.).
  • At least one year of paid, formal (non-family care giving setting) experience providing case management services and working with adults with behavioral health, mental health, and/or addiction issues.
  • Ability to work independently, stay organized, and meet deadlines.
  • Valid Wisconsin Driver's License, access to a vehicle, personal motor vehicle insurance meeting the minimum requirements of the Agency, passing of a Department of Justice Background Check, and Federal Sex Offender Registry Check.
