Caregiving Specialist

We are seeking a dedicated Caregiving Specialist to join our team at Cambridge Health Alliance (CHA). As a Caregiving Specialist, you will play a vital role in supporting the therapeutic needs of program participants. This is an exciting opportunity to make a meaningful difference in the lives of our patients.

About the Role

This position focuses on collaborating with an interdisciplinary team to provide comprehensive care to program participants. You will be responsible for evaluating and treating participants in their homes, as well as at our Adult Day Health Center. Your expertise will help us deliver quality care, promote service excellence, and foster cultural responsiveness.

Responsibilities

  • Evaluate and treat program participants in their homes and at the PACE Center Clinic
  • Develop and implement individualized care plans to meet the unique needs of each participant
  • Collaborate with an interdisciplinary team to ensure seamless care coordination
  • Maintain current American Heart Association or American Red Cross BLS for Healthcare Providers certification
  • Bachelors, Masters, or Doctoral Degree in Physical Therapy required
