About the Position:

The Permanency Coordinator will work closely with our dedicated Clinical Director to provide comprehensive, trauma-informed services that prioritize the well-being and permanency objectives of our youth.

Main Responsibilities:

  • Developing and implementing individualized transition plans for our students
  • Providing ongoing support and guidance to students, families, and staff
  • Collaborating with community resources to ensure seamless coordination of services

Requirements:

A Bachelor's Degree in Human Services, Social Work, or Criminal Justice, as well as a valid driver's license and minimum age of 21.
