Job Summary

The Licensed Clinical Social Worker - Geriatric Mental Health Specialist is responsible for providing initial evaluations and individual and group therapy to patients. This role requires a strong clinical knowledge base and the ability to work independently. The ideal candidate will have experience in a primary healthcare setting and be bilingual in Spanish.

Key Responsibilities
  • Conduct initial evaluations and provide individual and group therapy to patients
  • Provide care coordination and management to patients with social determinants of health challenges
  • Collaborate with physicians and other patient care team members to ensure comprehensive care
  • Ensure accurate and timely documentation of patient progress
  • Stay up-to-date with continuing education requirements and best practices in geriatric mental health
Requirements
  • Master's degree in social work
  • Current licensure as a Licensed Clinical Social Worker with at least three years of behavioral healthcare experience
  • Experience in a primary healthcare setting preferred
  • Bilingual in Spanish preferred
