Care Management Coordinator

The JobWe are seeking a highly organized and detail-oriented Care Management Coordinator to support our Integrated Care Case Managers in the day-to-day clinical operations. The successful candidate will be responsible for managing daily inpatient and skilled nursing census, confirming admissions and discharges, and reconciling census. They will also close cases for inpatient and skilled nursing discharges, send out appropriate documents to patients and primary care physicians, and support the coordination of timely delivery of last day cover letters to skilled nursing facilities.
Key Responsibilities- Manage daily inpatient and skilled nursing census
- Confirm admissions and discharges
- Reconcile census
- Closing cases for inpatient and skilled nursing discharges
- Send out appropriate documents to patients and primary care physicians
- Support the coordination of timely delivery of last day cover letters to skilled nursing facilities
- Review tasks assigned by Integrated Care Nurse Practitioners and coordinate member needs accordingly
- Support the coordination of direct admissions to skilled nursing facilities from assisted living facilities or board care settings
- Track patients referred to hospice care and assign tasks in EHR
- High School Diploma or GED
- 2+ years of experience as a care coordinator in a care management department
- Ability to communicate effectively verbally and in writing
- Highly organized with attention to detail
- Strong interpersonal skills, hospitality, and change agent mindset
- Adaptable and open to change
- Base salary range: $23.51 - $30.00 / hour
