Position: Medical Assistant Care Coordinator

Overview: The Medical Assistant Care Coordinator collaborates closely with the Chronic Care Coordination Manager to oversee patient health conditions, follow up on care directives from healthcare providers, and facilitate uninterrupted care for patients. This role involves engaging with patients and their families through phone and email communications to guarantee thorough care.

Qualifications:

  • High School Diploma or equivalent
  • Certification as a Medical Assistant (CMA) or equivalent, or valid Licensed Practical Nurse (LPN) license
  • A minimum of 2 years of recent clinical experience in a healthcare setting

Key Responsibilities:

  • Conduct telephonic outreach to patients managing chronic illnesses
  • Develop patient-centered care plans aligned with health objectives
  • Gather health-related information through patient outreach efforts
  • Provide education and support for patient self-management strategies
  • Coordinate care with healthcare professionals and other service providers
  • Ensure precise documentation within electronic health records

Essential Requirements:

  • Comprehensive medical knowledge regarding the clinical needs of patients with chronic conditions
  • Excellent verbal and written communication abilities
  • Capability to work independently
  • Proficient computer skills

Work Environment: Primarily situated in a well-lit office setting with minimal local travel necessary.

Mental/Physical Requirements: Responsibilities primarily involve verbal communication and sitting at a desk while utilizing a computer.
