**Key Responsibilities**
- Assist patients and their families in understanding the social and emotional factors that impact their health status and develop coping mechanisms to manage these challenges.
- Collaborate with the healthcare team to develop comprehensive care plans that address the unique needs of each patient.
- Provide education and support to patients and their families on community resources available to assist with healthcare needs.
- Function as a Bereavement Coordinator, providing support and services to patients and their families after the loss of a loved one.
**Requirements**
- Master's Degree in Social Work from a Council of Social Work Education-accredited program.
- One year of experience in a healthcare setting, with a focus on social work.
- Current CPR certification and a valid driver's license.
- Ability to work independently and as part of a team to provide exceptional patient care.
