Position Overview

Role: Medical Assistant Care Coordinator

Organization: Center for Primary Care

Overview: The Medical Assistant Care Coordinator collaborates closely with the Chronic Care Coordination Manager to oversee patient health conditions, follow up on care directives from healthcare providers, and facilitate uninterrupted patient care. This role involves engaging with patients and their families through phone and email communications to guarantee thorough care delivery.

Key Qualifications:

  • High School Diploma or equivalent
  • Certification as a Medical Assistant (CMA) or equivalent, or valid Licensed Practical Nurse (LPN) credentials
  • At least 2 years of recent clinical practice experience

Core Responsibilities:

  • Conduct telephonic outreach to patients managing chronic conditions
  • Develop patient-centered care plans aligned with health objectives
  • Gather health-related information through patient interactions
  • Offer education and support for patient self-management strategies
  • Coordinate care efforts with medical personnel and other healthcare providers
  • Ensure precise documentation within electronic health records

Essential Requirements:

  • In-depth medical knowledge regarding the clinical needs of patients with chronic illnesses
  • Excellent verbal and written communication abilities
  • Capability to work independently
  • Proficient computer skills

Work Environment: Primarily situated in a well-lit office setting with minimal local travel required.

Mental/Physical Demands: Responsibilities mainly involve verbal communication and sitting at a desk while utilizing a computer.
