Inpatient Rehabilitation Facility Coordinator

The Inpatient Rehabilitation Facility Coordinator plays a vital role in the meticulous management, completion, and submission of the Inpatient Rehabilitation Facility Patient Assessment Instrument (IRF-PAI) in collaboration with interdisciplinary team members, ensuring accuracy and compliance with established timelines. This position also involves educating staff on the effective use of Care Tool and documentation practices for GG coding, alongside assisting in data collection and analysis for performance improvement initiatives.

Primary Duties:

  • Effectively manage and prioritize multiple projects.
  • Exhibit strong time management capabilities.
  • Utilize Excel proficiently, including the creation of tables, basic formulas, and graphs.
  • Demonstrate knowledge of the UDS data management system.
  • Utilize GG coding competently with Care Tool.
  • Apply pressure ulcer staging appropriately for IRF-PAI documentation.
  • Establish and maintain systems for data collection related to IRF-PAI and performance improvement projects.
  • Participate in hospital committee meetings as designated by the Program Director.
  • Serve as a resource for the Rehabilitation unit and hospital staff.
  • Complete all required competencies for the hospital and rehabilitation department within specified timeframes.
  • Attend a percentage of mandatory rehabilitation unit meetings and training sessions.
  • Gather and analyze data from Medical Records to fulfill IRF-PAI requirements.
  • Review IRF-PAI submissions for accuracy prior to finalization.
  • Collaborate with the Program Director to address any data discrepancies or timing issues related to IRF-PAI submissions.
  • Coordinate IRF-PAI processes with various departments to ensure timely billing and proper documentation retention.
  • Submit CMGs to the billing office to facilitate accurate billing.
  • Ensure IRF-PAI data is accurately recorded in collaboration with medical records personnel.
  • Organize ongoing education for therapy and nursing staff, including necessary competency assessments.
  • Identify and rectify discrepancies in Care Tool documentation in real-time.
  • Report non-compliance issues to the Program Director.
  • Assist the Rehab Director and Medical Director by providing necessary outcome reports.
  • Monitor trends in patient care through the IRF-PAI process.
  • Support the ongoing management of performance improvement processes and data collection.
  • Facilitate communication of outcome data to the interdisciplinary team.
  • Assist in the insurance authorization process for patients in the rehabilitation facility.
  • Track compliance and coordinate interdisciplinary team meetings.
  • Maintain professionalism and ethical standards in all communications with staff while pursuing continuous learning and knowledge sharing.

Qualifications

Educational Background: An Associate's Degree in Nursing is required; a Bachelor of Science in Nursing (BSN) is preferred.

Experience: A minimum of 2 years in a clinical or hospital environment with a solid understanding of clinical conditions. Experience in rehabilitation or skilled nursing settings is advantageous.

Licenses and Certifications: A current RN license in South Carolina is mandatory. Candidates must pass the Quality Improvement credentialing exam bi-annually.

Skills and Competencies: Strong written and verbal communication skills are essential. Candidates must demonstrate the ability to work independently, possess critical thinking skills, and exhibit strong organizational capabilities. Proficiency in computer applications, including word processing, spreadsheets, and databases, is required.
