Job Summary

We are seeking a highly motivated Care Navigator to join our team at Sloan's Lake Primary Care Clinic. As a Care Navigator, you will play a critical role in ensuring seamless transitions of care for patients, addressing barriers to care, and facilitating patient compliance with ambulatory care, HIV care, and other health services.

Key Responsibilities:
  • Manage transitions of care for patients from ED, inpatient, SNF, specialty to primary care
  • Address barriers to care, improve patient experience, and connect patients to care as appropriate
  • Maintain ongoing tracking and documentation on referrals to promote team awareness and ensure patient safety
  • Assemble patient clinical background and referral needs, providing appropriate clinical information as applicable
  • Contact review organizations and insurance companies to ensure prior approval requirements are met
  • Act as an extender for the care team, assisting patients with problem-solving potential issues related to the healthcare system, financial or social barriers
  • Identify and utilize cultural and community resources, serving as a liaison for internal and external care providers and/or families/caregivers
  • Track individual patient referral process, ensuring appointment scheduling, necessary follow-up, and barrier addressing
  • Use motivational interviewing and teach-back tools to assist patient understanding of care plans
Requirements:
  • High School Diploma or GED required
  • 1-3 years experience in a clinical care setting required
