Job Summary

Denver Health is seeking a skilled Care Navigator/Case Manager to provide patient navigation services and facilitate care coordination for patients in our healthcare system.

Key Responsibilities:
  • Manage transitions of care for patients from emergency departments, inpatient settings, and specialty care to primary care.
  • Address barriers to care coordination, improve patient experience, and connect patients to necessary resources and services.
  • Maintain accurate tracking and documentation of referrals to ensure team awareness and patient safety.
  • Assemble patient clinical background and referral needs, providing relevant clinical information as applicable.
  • Contact review organizations and insurance companies to ensure prior approval requirements are met.
  • Act as an advocate for patients and families, addressing potential issues related to healthcare systems, financial, or social barriers.
  • Identify and utilize cultural and community resources, serving as a liaison for internal and external care providers and families/caregivers.
  • Collaborate with care teams to implement solutions surrounding patient care coordination, including scheduling appointments, transportation, and interdisciplinary communication.
Requirements:
  • High School Diploma or GED required.
  • 1-3 years of experience in a clinical care setting required.
Preferred Qualifications:
  • Intermediate knowledge of correctional and/or justice department(s).
  • Intermediate knowledge of community resources needed for reentry at Denver Health's integrated care system.
  • Bilingual in English/Spanish preferred.
  • Knowledge of Epic and other electronic health records systems.
  • Knowledge of Microsoft Office suite products.
