Job Summary

We are seeking a highly skilled Care Navigator to join our team at Westside Adult Medical Clinic. As a Care Navigator, you will play a critical role in ensuring seamless transitions of care for patients, addressing barriers to care, and facilitating patient compliance with ambulatory care, HIV care, and other essential services.

Key Responsibilities
  • Manage transitions of care for patients from ED, inpatient, SNF, specialty to primary care
  • Address barriers to care, improve patient experience, and connect patients to care as appropriate
  • Maintain ongoing tracking and documentation of referrals to promote team awareness and ensure patient safety
  • Assemble patient clinical background and referral needs, providing appropriate clinical information as applicable
  • Contact review organizations and insurance companies to ensure prior approval requirements are met
  • Act as an extender for the care team, assisting patients in problem-solving potential issues related to the healthcare system, financial or social barriers
  • Act as the system navigator and point of contact for patients and families, with patients and families having direct access for asking questions and raising concerns
  • Identify and utilize cultural and community resources, serving as a liaison for internal and external care providers and/or families/caregivers
  • Identify opportunities and collaborate with the care team to implement solutions surrounding patient care coordination
Requirements
  • High School Diploma or GED Required
  • 1-3 years Experience in a clinical care setting Required
Preferred Qualifications
  • Intermediate knowledge of the correctional and/or justice department(s)
  • Intermediate knowledge of the community resources needed for reentry at Denver Health's integrated care system
  • Ability to support community engagement to connect persons involved in the criminal justice system and Denver Health
  • Strong oral and written communication skills
  • Proven ability to work effectively, both individually and as a team, with populations experiencing disadvantages, persons involved with the criminal justice system, persons experiencing homelessness, and recent immigrants
  • Must be empathetic to the needs of all patients
  • Intermediate knowledge of the community resources needed to support individuals in the Denver Metro area
  • Subject matter expert for various community advisory councils that serve to better connect corrections and Denver Health
  • Bilingual in English/Spanish preferred
  • Knowledge of Epic and other electronic health records systems
  • Knowledge of Microsoft Office suite products
