Job Summary

The Patient Navigator II plays a vital role in facilitating access to healthcare services, social support, and community resources for families. This position serves as an intermediary between the healthcare system, social services, and the community, ensuring that patients receive timely treatment and necessary care.

Key Responsibilities
  • Client and Provider Outreach: Engage in outreach activities to educate families about Medicaid and CHP+ benefits, including well-child and oral health care. Provide administrative case management and referral management services to ensure seamless care coordination.
  • Community Outreach: Establish and maintain relationships with community partners, providing outreach and education activities at community events. Plan and coordinate collaborative efforts to ensure better service delivery and education to the population served.
  • Patient Care Coordination: Assist with administrative and clerical duties related to patient care coordination and referral management. Help clients schedule appointments and transportation needs, and follow up with members who missed their appointments.
  • Case Management: Liaise between healthcare providers and patients, assisting with the overall program navigation of Medicaid and CHP+ medical assistance programs. Help clients with billing questions or issues regarding program benefits.
  • Data and Reporting: Enter patient information, review medical records, and maintain reports as needed.
Requirements
  • High School Diploma or GED Required
  • 1-3 years of clerical/front/back office experience in a medical setting Required
