We are seeking a highly motivated and detail-oriented Care Navigator/Case Manager to join our team at Denver Health. As a key member of our healthcare team, you will play a critical role in ensuring seamless transitions of care for our patients, addressing barriers to care, and promoting patient engagement and retention.
Key Responsibilities:- Manage transitions of care for patients from emergency departments, inpatient settings, skilled nursing facilities, and specialty care to primary care.
- Identify and address barriers to care, including financial, social, and cultural challenges, to facilitate care coordination and support plans of care.
- Provide patient education and support to promote health literacy and self-management skills.
- Collaborate with healthcare teams to ensure timely and effective care coordination, including scheduling appointments, arranging transportation, and communicating with patients and families.
- Utilize motivational interviewing and teach-back techniques to enhance patient understanding of care plans and promote adherence to treatment recommendations.
- Track patient referral processes, including appointment scheduling, follow-up care, and barrier resolution.
- High school diploma or equivalent required; associate's or bachelor's degree in a healthcare-related field preferred.
- 1-3 years of experience in a clinical care setting, preferably in care coordination or patient navigation.
- Intermediate knowledge of community resources and healthcare systems, including Epic and other electronic health records systems.
- Strong communication and interpersonal skills, with ability to work effectively with diverse populations and healthcare teams.
- Bilingual in English/Spanish preferred.
- Competitive hourly rate of $32.26.
