Plymouth Housing Group is seeking a skilled Housing Case Manager to join our team. As a Housing Case Manager, you will play a critical role in supporting residents in their housing stability and providing care coordination and supportive services to meet their goals.
Key Responsibilities:- Develop and maintain Housing Service Plans with residents to address transition needs and long-term housing stability needs.
- Maintain extensive knowledge of resources available to residents, including public benefits and community organizations.
- Assist residents in scheduling appointments, arranging transportation, and accompanying them to appointments as needed.
- Advocate, consult, and collaborate with community providers to ensure continuity of care.
- Mediate conflict using de-escalation and other techniques.
- Intervene in crises, respond to emergencies, and initiate action as required.
- Coordinate with the internal behavioral health team to support cohesive services to tenants.
- BA/BS degree in a human service field or equivalent experience.
- Minimum of two years of experience working with homeless and low-income individuals, including persons living with mental and physical illnesses, chemical dependency/addictions, disabilities, HIV/AIDS.
- Valid Washington State Driver License and insurable driving record.
- Experience working in a Permanent Supportive Housing environment.
