Job Summary

We are seeking a skilled Case Manager to join our team at the Low Income Housing Institute. As a Case Manager, you will play a critical role in assisting formerly homeless individuals to stabilize in permanent supportive housing. This is a unique opportunity to make a meaningful difference in the lives of our clients and contribute to the development of a supportive community.

Key Responsibilities
  • Build effective relationships with tenants to provide support and guidance in achieving their goals.
  • Provide information and referral services to residents, including distribution of written materials and maintenance of updated information tools.
  • Assess tenants' current level of functioning and social service needs, and offer case management services as needed.
  • Make referrals or initiate services to address mental health, substance abuse, and disability issues.
  • Coordinate with local agencies to provide services that meet the needs of LIHI residents.
  • Work collaboratively with building management and Supportive Services staff to increase stability and retention of tenants in LIHI housing.
Requirements
  • Bachelor's degree in Social Work or related field, or Associate degree with 3 years of experience.
  • At least two years of experience working with a diverse population.
  • Ability to work well with community agencies and resources.
  • Excellent written and verbal skills.
  • Current Washington State driver's license.
  • Computer experience.
  • Must pass background check and drug screening test.
