- Collaborate with interdisciplinary teams to deliver high-quality patient care.
- Utilize the nursing process and current standards of nursing practice to provide exceptional hospice care.
- Remain competent in technical nursing skills, including IVs and G-tubes.
- Provide effective patient and family teaching, promoting self-care and empowerment.
- Communicate with physicians and CAH staff regarding patient status and care plans.
- Understand family dynamics and work effectively within diverse family systems.
- Demonstrate ability to act as a team player, providing support and mentoring to peers as needed.
- Utilize resources appropriately, ensuring efficient use of agency resources.
- Demonstrate patient and family advocacy, providing information on available resources and support services.
- Accurately identify and address patient needs for additional services.
- Perform an adequate number of nursing visits to provide superior hospice care, meeting CAH standards.
- Communicate patient reports and status changes concisely and effectively.
- Perform and document HHA supervision every 14 days.
- Update Care Plan, Medication Profile, and HHA Treatment Plan routinely and as patient condition changes.
- Complete all nursing documentation per procedures and nursing standard of practice.
- Perform in a professional manner at all times, upholding agency standards and expectations.
- Manage caseload efficiently and effectively, prioritizing patient needs.
- Involves Clinical Manager and Medical Director in situations as needed.
- Follow guidelines for weekend and evening coverage, ensuring continuity of care.
- Understand and maintain professional boundaries with patients and families.
- Utilize good customer service skills internally and externally, promoting a positive agency image.
- Assist in orientation of new staff, providing support and guidance as needed.
- Pursue standards of excellence in performance, striving for continuous improvement.
- Work with Clinical Manager to identify opportunities for improvement and address them.
- Proactively increase knowledge of hospice care and standards of practice, staying current with industry developments.
- Assist in hospice education in the community, promoting awareness and understanding of hospice care.
- Participate in achieving CAH goals and objectives, contributing to agency success.
- Represent CAH and promote hospice care in the community and among healthcare professionals.
- Demonstrate knowledge and understanding of corporate compliance and regulatory requirements.
- Ensure compliance with guidelines set forth by regulatory agencies and agency policies and procedures.
- Practice confidentiality principles, adhering to agency and federal HIPAA/HITECH guidelines.
- Participate in all mandatory inservices, staying current with agency policies and procedures.
- Complete other duties as assigned, upholding agency standards and expectations.
