About the Role

We are seeking a skilled and compassionate Community Hospice Aide to join our team at Tufts Medicine Care at Home. As a member of our team, you will provide high-quality, patient-centered care to individuals in their homes, assisted living facilities, and skilled nursing facilities.

Key Responsibilities
  1. Provide personal care and support to patients, including bathing, dressing, grooming, and ambulation/mobility.
  2. Assist with bowel and bladder training, catheter care, ostomy care, and adaptive equipment as designated in the patient's care plan.
  3. Identify and report changes in patient mobility, weakness, sleepiness, and fatigue to the supervising clinician.
  4. Provide emotional support and companionship to patients and their families.
  5. Collaborate with the healthcare team to ensure seamless care transitions and optimal patient outcomes.
Requirements
  • High school diploma or equivalent.
  • Certified Home Health Aide or Licensed Nursing Aide.
  • Valid driver's license.
