The Clinical Coordinator - Home Health RN provides a supportive role to the clinical management team, performing a variety of duties in the coordination of care delivery. Key responsibilities include:

  • Triaging calls related to patient care, including physician orders and staff issues.
  • Providing effective coordination of clinical services, consistent with the objectives of the agency.
  • Working closely with clinical staff to provide necessary services for patients and families in a timely manner.
  • Communicating regularly with Clinical Managers and the Clinical Director on clinical practice and performance-related issues.
  • Demonstrating expertise and knowledge in areas such as current methods of care and treatment, policies, and protocols.
  • Assisting with individual caseload reviews and audits as assigned by the manager or Clinical Director.
  • Providing documentation that is concise, accurate, and informative, supporting reimbursement.
  • Practicing in accordance with standards of care, policies, and scope of practice, demonstrating awareness of regulatory guidelines.

Minimum Qualifications:

  • Registered Nurse (RN)
  • Three (3) years of community health experience required or a combination of at least one year of acute medical/surgical nursing experience and 2 years of home health experience required.
