Job Summary

Tufts Medicine Care at Home is seeking a skilled RN Weekend Clinical Coordinator to join our team. As a key member of our clinical management team, you will play a vital role in the coordination of patient care delivery in the comfort of their own homes.

Key Responsibilities

  • Triage calls related to patient care, including physician orders and staff issues.
  • Work closely with clinical staff to provide necessary services for patients and families in a timely manner.
  • Communicate regularly with Clinical Managers and the Clinical Director on clinical practice and performance-related issues.
  • Demonstrate expertise and knowledge in areas such as current methods of care and treatment, policies, and protocols.
  • Assist with orientation and preceptorship of clinical staff.
  • Evaluate performance of clinical staff in relation to established standards.
  • Determine staff competency in a fair and consistent manner.
  • Triage calls from physicians and follow through on required actions.
  • Provide documentation that is concise, accurate, and informative.

Requirements

Three years of community health experience required or a combination of at least one year of acute medical/surgical nursing experience and two years of home health experience.
