Hospice Case Manager

We are seeking a skilled and compassionate RN Case Manager to join our team at Della Infotech. As a key member of our hospice care team, you will provide high-quality nursing care to patients and their families, utilizing current principles of palliative care and symptom management.
Key Responsibilities:- Provide hospice nursing care to patients and families, utilizing current principles of palliative care and symptom management.
- Coordinate care in a cost-efficient manner, ensuring the best possible outcomes for our patients.
- Collaborate effectively with other members of the Interdisciplinary Team (IDT) to deliver comprehensive care.
- Utilize the nursing process and current standards of nursing practice to provide exceptional hospice care.
- Remain competent in technical nursing skills, including IVs and G-tubes.
- Provide effective patient and family teaching, empowering them to take an active role in their care.
- Communicate with physicians and CAH staff regarding changes in patient condition, ensuring seamless care coordination.
- Understand family dynamics and work effectively within various types of family systems, providing support and guidance as needed.
- Demonstrate the ability to act as a team player, providing peers with support and mentoring when appropriate.
- Utilize resources appropriately, ensuring the best possible outcomes for our patients.
- Demonstrate the ability to act as a patient and family advocate, providing information about available resources and supporting their needs.
- Accurately identify the need for additional services, ensuring our patients receive the care they need.
- Perform an adequate number of nursing visits to provide superior hospice care, meeting CAH standards.
- Communicate patient reports and status changes concisely and effectively, ensuring seamless care coordination.
- Perform and document HHA supervision every 14 days, ensuring compliance with regulations.
- Update Care Plans, Medication Profiles, and HHA Treatment Plans routinely and as patient condition changes, ensuring accurate and up-to-date information.
- Complete all nursing documentation per procedures and nursing standards of practice, including assessments, interventions, responses to interventions, communications, verbal orders, etc.
- Perform in a professional manner at all times, upholding the highest standards of care and compassion.
- Manage caseload efficiently and effectively, ensuring timely and high-quality care.
- Involves Clinical Manager and Medical Director in situations appropriately, ensuring seamless care coordination.
- Follow guidelines for weekend and evening coverage, ensuring our patients receive the care they need.
- Understand how to maintain professional boundaries with patients and families, ensuring a safe and respectful environment.
- Utilize good customer service skills internally and externally, providing exceptional care and support.
- Assist in orientation of new staff, ensuring a smooth transition into our team.
- Pursue standards of excellence in performance as a hospice case manager, continuously improving and refining our care.
- Work with Clinical Manager to identify opportunities for improvement in performance and work to address them, ensuring continuous growth and development.
- Proactive in increasing knowledge of hospice care and standards of practice, staying up-to-date on the latest developments and best practices.
- Assist in hospice education in the community, promoting awareness and understanding of our services.
- Participate in achieving CAH goals and objectives, ensuring our team's success and growth.
- Represent CAH and promote hospice care in the community and other healthcare professionals, raising awareness and understanding of our services.
- Demonstrate a knowledge and understanding of what to report to the CEO or Chief Compliance Officer when concerns of corporate compliance arise, ensuring compliance with regulations.
- Ensure compliance within guidelines set forth by regulatory agencies (DPH, ERISA, etc.) and demonstrate compliance with Home Health Foundation policies and procedures.
- Practice confidentiality principles set by the agency and federal HIPAA/HITECH guidelines, ensuring the protection of patient information.
- Participate in all mandatory inservices, ensuring our team stays up-to-date on the latest developments and best practices.
- Complete other duties as assigned, ensuring our team's success and growth.
