About the Role

AgeSpan is seeking a dedicated and compassionate Geriatric Support Services Coordinator to join our team. As a key member of our interdisciplinary team, you will play a vital role in ensuring the health and independence of our elderly clients.

Key Responsibilities:

  • Conduct comprehensive health and functional assessments for seniors enrolled in our Senior Care Organizations (SCO).
  • Develop and monitor personalized service plans to promote overall health and independence.
  • Collaborate with our interdisciplinary team to ensure effective coordination and delivery of care in the client's home.

Requirements:

  • BA/BS in social work, human services, or a related field, plus experience working with elders in a healthcare setting.
  • Strong written, verbal, and computer skills, with the ability to work effectively in a team environment.
  • Valid driver's license and reliable transportation.
