As a leading behavioral health organization, Connections Health Solutions specializes in crisis stabilization and access to necessary care for those requiring behavioral health services. We are currently seeking a Licensed Practical Nurse (LPN) for our Crisis Response Center (CRC). In this role, you will play a vital part in consumer care through medication administration, documentation, and ongoing assessments, collecting data on both behavioral and physical health, as well as substance use history. The LPN will also provide essential education to consumers upon admission and discharge regarding their behavioral health diagnoses and prescribed medications to encourage adherence to treatment plans.

Key Responsibilities
  • Assist in the Nursing Assessment, ongoing evaluations, and health history documentation under RN supervision. Collaborate with RNs on the consumer's care plan and communicate relevant information such as blood glucose levels and CIWA/CINA scores.
  • Accurately transcribe Provider orders and administer medications as directed. Ensure patients sign medication consent forms and perform chart checks, documenting medications prescribed. Conduct lab draws and provide wound care as necessary, recording all medications brought in by patients on the reconciliation/storage form.
  • Support Behavioral Health Specialists in managing the environment and gathering insights into the psychopharmacological and physiological needs of patients.
  • Educate consumers, families, and involved parties about physical health, psychiatric diagnoses, treatment options, and psychopharmacology.
  • Utilize appropriate verbal interventions to de-escalate potentially dangerous situations, demonstrating a solid understanding of therapeutic techniques and safety protocols.
  • Administer emergency medications as required during seclusion/restraint situations, showcasing comprehensive knowledge of psychotropic medications.

Minimum Qualifications:

  • Graduation from an accredited Nursing Program with licensure from the Arizona Board of Nursing
  • Unrestricted Licensed Practical Nurse license through the Arizona Board of Nursing
  • A minimum of one year of experience in psychiatric, behavioral health, or a related field.
  • Compliance with the Company’s mandatory vaccination policy, including providing proper documentation or qualifying for an exemption prior to employment.
