As a behavioral health organization, Connections Health Solutions specializes in crisis stabilization and facilitating access to care for those in need of behavioral health services. We are currently seeking a Licensed Practical Nurse (LPN) for our Crisis Response Center (CRC). This role is vital in enhancing consumer care through medication management, thorough documentation, transcription of provider orders, and ongoing assessments of consumers, which includes gathering data on both behavioral and physical health as well as substance use history. The LPN will also provide essential education to consumers during their admission and discharge regarding their behavioral health diagnoses and prescribed medications to promote adherence to treatment plans.
Key Responsibilities- Assist in compiling data for nursing assessments, ongoing evaluations, and health histories under the supervision of a registered nurse (RN). Collaborate with the RN on the consumer's care plan and communicate significant information such as blood glucose levels, CIWA/CINA scores, and breathalyzer results.
- Transcribe provider orders and administer medications as directed. Ensure patients sign medication consent forms and perform chart checks, confirming accuracy as a second reviewer on prescribed medications. Collect lab samples and provide wound care as directed by providers when necessary. Maintain accurate records of all medications brought in by patients on the medication reconciliation/storage form.
- Support Behavioral Health Specialists in managing the environment and gathering information regarding the psychopharmacological and physiological needs of patients.
- Educate consumers, families, and other stakeholders about physical health, psychiatric diagnoses, treatment options, and psychopharmacology.
- Utilize appropriate verbal interventions to de-escalate potentially dangerous situations. Demonstrate a thorough understanding of therapeutic techniques and the ability to articulate roles and responsibilities related to safety.
- Administer emergency medications as prescribed during seclusion/restraint situations, showcasing a comprehensive knowledge of psychotropic medications.

Minimum Qualifications:
- Graduation from an accredited nursing program with licensure from the Arizona Board of Nursing
- Unrestricted licensure as a Licensed Practical Nurse through the Arizona Board of Nursing
- A minimum of one year of experience in psychiatric, behavioral health, or a related field.
- Compliance with the company's mandatory vaccination policy, requiring all successful applicants to be fully vaccinated and provide appropriate documentation or seek exemption through the company's process prior to starting employment.
