As a leading behavioral health organization, Connections Health Solutions offers crisis stabilization and care access for those in need of behavioral health services. We are currently seeking a Licensed Practical Nurse (LPN) for our Crisis Response Center (CRC). In this role, you will play a vital part in consumer care through medication administration, documentation, transcription of provider orders, and ongoing assessments by gathering data on both behavioral and physical health, as well as substance use history. You will also provide education to consumers during their admission and discharge regarding their behavioral health diagnoses and medications to enhance treatment compliance.

Key Responsibilities
  • Assist in the Nursing Assessment, ongoing re-assessment, and health history documentation under RN supervision. Collaborate with RNs on the consumer's care plan and report relevant information such as blood glucose levels, CIWA/CINA scores, and breathalyzer results.
  • Transcribe provider orders and administer prescribed medications. Ensure patients sign medication consent forms and perform chart checks, signing off as a second reviewer on prescribed medications. Conduct lab draws and provide wound care as ordered by providers when necessary. Accurately document all medications brought in by patients on the medication reconciliation/storage form.
  • Support Behavioral Health Specialists in managing the environment and gathering information regarding the psychopharmacological and physiological needs of patients.
  • Educate consumers, families, and involved parties about physical health, psychiatric diagnoses, treatment options, and psychopharmacology.
  • Utilize appropriate verbal interventions to de-escalate potentially dangerous behaviors. Demonstrate proficiency in therapeutic techniques and articulate your roles and responsibilities related to safety.
  • Administer emergency medications as directed during seclusion/restraint situations. Exhibit comprehensive knowledge of psychotropic medications.

Minimum Qualifications:

  • Graduation from an accredited Nursing Program with licensure from the Arizona Board of Nursing
  • Unrestricted Licensed Practical Nurse license through the Arizona Board of Nursing
  • A minimum of one year of experience in psychiatric, behavioral health, or a related field.
  • The Company enforces a mandatory vaccination policy. All successful candidates must be fully vaccinated, including providing appropriate documentation, or must qualify for an exemption as per the Company's exemption process prior to employment commencement.
