Key Responsibilities

The Advanced Practice Nurse in Palliative Care delivers direct care to patients and families through specialized consultations, ensuring essential services for individuals facing serious illnesses with a prognosis of one year or less who are not yet ready for hospice care.
  • Utilizes approved evidence-based practices in palliative medicine to provide ethical and effective patient care, addressing physical, emotional, spiritual, psychosocial, ethical, educational, and safety needs, tailored to the patient demographics served.
  • Exhibits clinical expertise by applying the principles of palliative care to develop comprehensive care plans.
  • Systematically collects relevant information from various sources, including medical records, family histories, and caregiver insights, conducting thorough assessments of patients' physical, emotional, psychosocial, and spiritual health.
  • Completes detailed assessments utilizing palliative care domains, interprets findings, performs medication reconciliation, formulates problem-related diagnoses, initiates care plans with therapeutic interventions, and evaluates outcomes for effectiveness.

Qualifications

Experience Requirements

  • A minimum of five years of nursing experience, with a focus on clinical care in oncology, hospice, geriatrics, or intensive care preferred.

Licensure Requirements

  • Current certification as a Clinical Nurse Specialist or Nurse Practitioner in the state of practice.
  • Certification from a recognized national body such as ANCC, NBCHPN, or AANP.
  • Possession of a valid driver's license and vehicle insurance, with access to reliable transportation.
  • Current CPR certification is mandatory.

Preferred Licensure

  • Palliative Care ARNP certification is desirable.

Skill Requirements

  • Strong understanding of the interdisciplinary team approach.
