Advanced Practice Nurse in Palliative Care

Key Responsibilities
The Palliative Care Nurse Practitioner/APRN delivers direct care to patients and families through specialized palliative care consultations, ensuring comprehensive services for individuals facing serious illnesses with a prognosis of one year or less who may not yet qualify for hospice care.
- Utilizes evidence-based palliative medicine practices effectively and ethically in all aspects of patient care, considering physical, emotional, spiritual, psychosocial, ethical, educational, and safety factors relevant to the patient demographics served.
- Demonstrates clinical expertise by applying palliative care knowledge to develop effective care plans.
- Systematically collects relevant information from diverse sources, including medical records, family histories, and caregiver insights, conducting thorough assessments of patients' physical, emotional, psychosocial, and spiritual needs.
- Completes detailed assessments using palliative care domains, interprets findings, conducts medication reconciliations, formulates problem-related diagnoses, initiates care plans with therapeutic interventions, and evaluates outcomes for effectiveness.
Experience Requirements
- A minimum of five years of nursing experience, with a focus on clinical care in oncology, hospice, geriatrics, or intensive care settings.
- Current certification as a Clinical Nurse Specialist or Nurse Practitioner in the applicable state.
- Certification from a recognized national body such as ANCC, NBCHPN, or AANP.
- Possession of a valid driver's license and vehicle insurance, along with access to reliable transportation.
- Current CPR certification is mandatory.
- Palliative Care ARNP certification is preferred.
- Strong understanding of the interdisciplinary team approach in healthcare.
