Key Responsibilities
The Advanced Practice Nurse in Palliative Care delivers direct care to patients and their families through specialized consultations, ensuring that individuals with serious illnesses and limited prognoses receive appropriate services without the need for hospice care.
- Utilizes approved evidence-based practices in palliative medicine to provide comprehensive patient care, addressing physical, emotional, spiritual, psychosocial, ethical, educational, and safety needs relevant to the patient population.
- Exhibits clinical expertise by applying palliative care knowledge to develop effective care plans.
- Systematically collects relevant information from diverse sources, including medical records, family histories, and caregiver insights, conducting thorough assessments of patients' physical, emotional, psychosocial, and spiritual health.
- Completes detailed assessments utilizing various palliative care domains, interprets findings, performs medication reconciliations, formulates diagnoses, initiates care plans with therapeutic interventions, and evaluates outcomes for effectiveness.
Experience Requirements
- A minimum of five years of nursing experience, preferably in oncology, hospice, geriatrics, or intensive care settings.
- Current certification as a Clinical Nurse Specialist or Nurse Practitioner in the relevant state.
- Certification from a recognized national body such as ANCC, NBCHPN, or AANP.
- A valid driver's license and vehicle insurance, along with access to reliable transportation.
- Current CPR certification is mandatory.
- Palliative Care ARNP certification is desirable.
- Strong understanding of the interdisciplinary team approach in healthcare.
