Job Summary

We are seeking a skilled Registered Nurse to serve as a Care Coordinator for Skagit and Whatcom Counties. As a key member of our primary care team, you will play a vital role in coordinating care services and supporting our members in their healthcare journey.

Key Responsibilities
  • Establish and maintain effective communication with members, caregivers, and healthcare providers to ensure seamless care coordination.
  • Collaborate with multiple organizations to coordinate benefits, services, and resources for our members.
  • Review member status and adjust care plans as necessary to achieve optimal health outcomes.
  • Perform administrative tasks, such as data collection and entry, to support care coordination efforts.
  • Participate in program effectiveness analysis and planning to drive continuous improvement.
  • Conduct member visits and rounding to assess needs and provide support.
  • Communicate member status and care plans with healthcare providers and caregivers.
  • Assist with other clinical and quality responsibilities, such as identifying gaps in care.
Requirements
  • Current unrestricted Registered Nurse license in Washington.
  • 3+ years of RN experience, with 1+ year of case management experience.
  • Experience with electronic medical records and Microsoft Office.
  • Proven excellent communication and customer service skills.
  • Valid driver's license and ability to travel locally 75% of the time.
Preferred Qualifications
  • BSN degree.
  • Experience working in post-acute care, such as long-term care or hospice, and/or case management experience as an RN.
  • Experience working with the geriatric population.
  • Familiarity with the long-term care business environment.
