RN Care Manager II, Special Needs, BSN

We are seeking a highly skilled RN Care Manager II to join our team in Philadelphia, Montgomery, Lower Bucks, or Delaware counties. As a Care Manager II, you will play a critical role in assisting members with complex care needs to achieve their optimal level of health.
Responsibilities- Assess members to determine care coordination and case management needs
- Develop and implement individualized care plans to address member needs
- Coordinate care and services with members, family members, PCPs, specialists, and facility/vendor providers
- Identify and implement appropriate interventions to address member issues
- Document progress towards meeting goals and resolving problems
- Must reside in Pennsylvania
- Qualified candidates will reside in Philadelphia, Montgomery, Lower Bucks, or Delaware counties
- Bachelor's Degree in Nursing or healthcare related field
- Current and unrestricted Registered Nurse licensure in Pennsylvania
- 3+ years of experience as a case manager preferred
- Minimum of 3 years of professional clinical practice experience as a RN required
