RN Care Manager II, Special Needs, BSN

We are seeking a highly skilled RN Care Manager II to join our team at AmeriHealth Caritas. As a key member of our care coordination program, you will play a vital role in ensuring our members receive the highest quality care and support.
Responsibilities- Assess members to determine care coordination and case management needs
- Develop and implement individualized care plans to address member goals and objectives
- Collaborate with healthcare providers and community resources to ensure seamless care coordination
- Provide education and support to members and their families to promote healthy behaviors and outcomes
- Document progress and outcomes in a timely and accurate manner
- BSN degree in Nursing or related field
- Current and unrestricted RN licensure in Pennsylvania
- 3+ years of experience in case management or a related field
- Proficiency in MS Office and electronic medical record systems
- Ability to travel within Philadelphia County as needed
