RN Care Manager II, Special Needs, BSN

We are seeking a skilled RN Care Manager II to join our team at AmeriHealth Caritas. As a key member of our care coordination program, you will play a vital role in ensuring our members receive the highest quality care and support.
Responsibilities- Assess members' needs and develop individualized care plans to achieve optimal health outcomes.
- Collaborate with healthcare providers and members to coordinate care and services.
- Identify and address barriers to care, including environmental, psycho-social, and support needs.
- Document progress and outcomes, ensuring accurate and timely reporting.
- Stay up-to-date with industry developments and best practices in case management and care coordination.
- Bachelor's Degree in Nursing or a related field.
- Current and unrestricted Registered Nurse licensure in Pennsylvania.
- 3+ years of experience in case management or a related field.
- Proficiency in MS Office, internet applications, and electronic medical record systems.
- Valid driver's license and car insurance.
