RN Care Manager II, Special Needs, BSN

We are seeking a highly skilled RN Care Manager II to join our team in Philadelphia County, PA. As a key member of our care coordination program, you will play a vital role in ensuring our members receive the highest quality care.
Responsibilities- Assess members to determine care coordination and case management needs
- Develop and implement individualized care plans to achieve optimal health outcomes
- Collaborate with members, providers, and other stakeholders to ensure seamless care coordination
- Identify and address barriers to care, including environmental, psycho-social, and support needs
- Document progress and outcomes to inform future care decisions
- Must reside within 30 minutes of Philadelphia County, PA
- Bachelor's Degree in Nursing or healthcare-related field
- Current and unrestricted Registered Nurse licensure in Pennsylvania
- 3+ years of experience as a case manager preferred
- Minimum of 3 years of professional clinical practice experience as a RN required
- Behavioral health experience, including managing care of individuals with mental health and substance use disorders
- Proficiency in MS Office, internet applications, and electronic medical record and documentation programs
- Eligible for Certification in Case Management within 12 months of accepting position
