About the Role

We are seeking a highly skilled RN Care Manager II to join our team in Philadelphia County, PA. As a key member of our care coordination program, you will play a vital role in ensuring our members receive the highest quality care.

Responsibilities
  • Assess members to determine care coordination and case management needs
  • Develop and implement individualized care plans to achieve optimal health outcomes
  • Collaborate with members, providers, and other stakeholders to ensure seamless care coordination
  • Identify and address barriers to care, including environmental, psycho-social, and support needs
  • Document progress and outcomes to inform future care decisions
Requirements
  • Must reside within 30 minutes of Philadelphia County, PA
  • Bachelor's Degree in Nursing or healthcare-related field
  • Current and unrestricted Registered Nurse licensure in Pennsylvania
  • 3+ years of experience as a case manager preferred
  • Minimum of 3 years of professional clinical practice experience as a RN required
  • Behavioral health experience, including managing care of individuals with mental health and substance use disorders
  • Proficiency in MS Office, internet applications, and electronic medical record and documentation programs
  • Eligible for Certification in Case Management within 12 months of accepting position
