RN Care Manager II, Special Needs, BSN

We are seeking a highly skilled RN Care Manager II to join our team at AmeriHealth Caritas Health Plan. As a key member of our care management team, you will play a vital role in ensuring our members receive high-quality, patient-centered care.
Responsibilities- Assess members' needs and develop individualized care plans to achieve optimal health outcomes.
- Collaborate with healthcare providers to coordinate care and services, ensuring seamless transitions and effective communication.
- Identify and address barriers to care, such as social determinants of health, to promote member engagement and empowerment.
- Provide education and support to members and their families to promote health literacy and self-management.
- Document progress and outcomes, using data to inform care decisions and improve member experiences.
- Bachelor's Degree in Nursing or a related field.
- Current and unrestricted Registered Nurse licensure in Pennsylvania.
- 3+ years of experience as a case manager or in a related role.
- Behavioral health experience, including managing care for individuals with mental health and substance use disorders.
- Proficiency in MS Office, internet applications, and electronic medical record systems.
- Eligible for Certification in Case Management within 12 months of accepting the position.
