Job Title: RN Care Manager II, Special Needs, BSNJob Summary

AmeriHealth Caritas is seeking a highly skilled RN Care Manager II to join our team. As a RN Care Manager II, you will be responsible for engaging members and providers to assess, plan, and establish individual member goals. You will facilitate and coordinate care for members while ensuring quality and cost-effective resources.

Responsibilities
  • Assess members to determine care coordination and case management needs
  • Complete comprehensive assessments of environmental, psycho-social, and support needs
  • Identify problems/barriers for care coordination and implement appropriate care management interventions
  • Create a plan of care to assist members in reducing/resolving problems and achieving their optimal level of health
  • Identify and implement the appropriate level of intervention based on the member's needs and clinical progress
  • Document progress towards meeting goals and resolving problems
  • Coordinate care and services with members, family members, PCP, specialists, and facility/vendor providers
Requirements
  • Must reside within 30 minutes of Philadelphia County, Pennsylvania
  • Bachelor's Degree in Nursing or healthcare-related field
  • Current and unrestricted Registered Nurse licensure in Pennsylvania
  • 3+ years of experience as a case manager preferred
  • Minimum of 3 years of professional clinical practice experience as a RN required
  • Behavioral health experience including managing care of individuals with mental health and substance use disorders, life stressors, and crises
  • Proficiency using MS Office, internet applications, and electronic medical record and documentation programs
  • Eligible for Certification in Case Management within 12 months of accepting position
