RN Care Manager II, Special Needs, BSN

AmeriHealth Caritas is seeking a highly skilled RN Care Manager II to join our team. As a key member of our care coordination program, you will work closely with members, providers, and other stakeholders to assess, plan, and establish individual member goals.
The ideal candidate will have a strong background in nursing, with a focus on case management and care coordination. You will be responsible for engaging members and providers to assess and plan care, facilitating and coordinating care to ensure quality and cost-effective resources, and serving as a single point of contact for members in the care coordination program.

Responsibilities- Assess members to determine care coordination and case management needs
- Complete comprehensive assessments of environmental, psycho-social, and support needs
- Identify problems and barriers for care coordination and implement appropriate care management interventions
- Create plans of care to assist members in reducing or resolving problems and achieving optimal health
- Identify and implement the appropriate level of intervention based on member needs and clinical progress
- Document progress towards meeting goals and resolving problems
- Coordinate care and services with members, family members, PCP, specialists, and facility/vendor providers
- Bachelor's degree in nursing or healthcare-related field
- Current and unrestricted registered nurse licensure in Pennsylvania
- 3+ years of experience as a case manager preferred
- Minimum of 3 years of professional clinical practice experience as a RN required
- Behavioral health experience, including managing care of individuals with mental health and substance use disorders, life stressors, and crises
- Proficiency using MS Office, internet applications, and electronic medical record and documentation programs
- Eligible for certification in case management within 12 months of accepting position
