We are seeking a highly skilled RN Care Manager II, Special Needs to join our team at AmeriHealth Caritas. As a key member of our care coordination program, you will play a vital role in ensuring our members receive the highest quality care and support.
Job Summary:The RN Care Manager II, Special Needs will be responsible for assessing the needs of our members and developing individualized care plans to address their unique challenges. You will work closely with our members, their families, and healthcare providers to ensure seamless care coordination and effective communication.
Key Responsibilities:- Assess members' needs and develop comprehensive care plans to address their unique challenges.
- Collaborate with members, families, and healthcare providers to ensure seamless care coordination and effective communication.
- Identify and implement appropriate interventions to address members' needs and improve their health outcomes.
- Document progress and outcomes to ensure continuous quality improvement.
- Participate in ongoing education and training to stay up-to-date on best practices and industry developments.
- Bachelor's degree in Nursing or a related field.
- Current and unrestricted Registered Nurse licensure in Pennsylvania.
- 3+ years of experience as a case manager, preferably in a healthcare setting.
- Behavioral health experience, including managing care of individuals with mental health and substance use disorders.
- Proficiency in MS Office, internet applications, and electronic medical record and documentation programs.
- Eligible for Certification in Case Management within 12 months of accepting position.
