Position Summary for Registered Nurse Case Manager (RNCM):
We have openings for Registered Nurse Case Managers in various locations.
In this role, you will plan, organize, and oversee home health care services, starting from initial evaluations and assessments to pain and medication management, and the development of care plans.
Your focus will be on ensuring a high-quality patient experience and achieving positive health outcomes through a detailed and personalized care plan, with proactive adjustments based on patient needs.
Responsibilities include:
- Preparing comprehensive and timely clinical documentation
- Coordinating care delivery with an interdisciplinary team, including physical therapists, occupational therapists, speech therapists, medical social workers, and home health aides
- Collaborating with the patient's primary care physician to maintain continuity of care
- Providing education and coaching to patients and their caregivers based on clinical diagnoses, empowering them to manage their conditions, prevent complications, and enhance their quality of life
Minimum Qualifications for Registered Nurse Case Manager (RNCM):
Candidates must possess a current California Registered Nurse (RN) license and a valid CPR card for Healthcare Providers. A minimum of one year of experience as a Registered Nurse (RN) in Home Healthcare is preferred.
Additionally, candidates must have a valid driver's license and an insured automobile that meets state or organizational requirements and is in good working condition.
Successful candidates will demonstrate excellent observational, problem-solving, verbal, and written communication skills, along with nursing competencies as per the established checklist. The ability to work independently with minimal supervision is essential.
