About the Role

We are seeking a highly skilled Community Resource Specialist to join our Integrated Care Management team at Mass General Brigham (Enterprise Services). As a key member of our team, you will play a vital role in connecting patients to resources, improving access to care, and supporting patients to achieve optimal health and wellbeing.

The Community Resource Specialist will work closely with our iCMP team leads to provide resource support to high-risk patient populations, acting as the subject matter expert for all resource needs in the patient's community. You will research, locate, and validate resources to support the care team and patients' needs, managing resource referral requests in the EMR and maintaining and updating the local iCMP resource database.

Key Responsibilities:

  • Partner with the iCMP care team to provide resource support to high-risk patient populations
  • Act as the subject matter expert for all resource needs in the patient's community
  • Research, locate, and validate resources to support the care team and patients' needs
  • Manage resource referral requests in the EMR
  • Maintain and update the local iCMP resource database
  • Perform outreach calls to patients for iCMP support and resource needs under the direction of the team lead
  • Participate in Case Reviews under the directive of the team lead and execute tasks as directed
  • Document all patient interactions in the systems EMR to ensure continuity of care and outreach tracking

Requirements:

  • High School Diploma or GED required; Bachelor's Degree or equivalent work experience preferred
  • Human services experience and knowledge of community resources preferred
  • Knowledge of hospital and healthcare systems preferred
  • Bilingual candidates encouraged but not required
