Part-Time Case Manager

We are seeking a highly skilled and compassionate Part-Time Case Manager to join our team at the City of Somerville. As a key member of our Health and Human Services department, you will play a vital role in providing case management services to residents in need of support for their mental, social, and physical wellbeing.
Key Responsibilities- Conduct assessments and intakes of clients to identify their needs and develop personalized plans to address their concerns.
- Develop relationships with community-based organizations and other governmental agencies to identify and provide access to resources and services for clients.
- Work collaboratively with staff from other City departments to coordinate services for clients and ensure a seamless referral process.
- Provide referrals to other community agencies for needs such as mental health, housing, homelessness, and substance use.
- Maintain accurate and up-to-date client databases to track efforts and outcomes, and produce required reports.
- Stay informed about social service resources offered by the City, State, and non-profit agencies, and develop strong partnerships with community organizations and service providers.
- Conduct workshops for internal City staff and external partner agencies on case management and oversight topics.
- Assist clients with navigating forms and systems to access programs and services.
- Assist with creating a seamless referral system to internal and external programs and services.
- Attend local coalition meetings on social service supports topics.
- Develop social media content, web content, and publicity materials in collaboration with the City of Somerville's Communications Department.
- Translate materials as needed.
- Bachelor's Degree and three to five (3-5) years' experience in case management services, or any equivalent combination of education, training, and experience.
- Licensed Social Workers (LSW) are strongly encouraged to apply.
- CPR/First Aid Certification within three months of hire and continually maintained thereafter.
- Ability to travel independently to multiple locations within the City to complete work assignments.
- Bi-lingual or multilingual ability in Spanish, Portuguese, and/or Haitian-Creole.
The work environment involves everyday discomforts typical of offices, with occasional exposure to outside elements. Noise or physical surroundings may be distracting, but conditions are generally not unpleasant.
Employee may be required to work beyond normal business hours in response to attend evening meetings or complete work assignments.

Few physical demands are required to perform the work. Work effort principally involves sitting to perform work tasks, with intermittent periods of stooping, walking, and standing. May also be some occasional lifting of objects such as office equipment and computer paper (up to 30 lbs.).
Duties are largely mental rather than physical, but the job may occasionally require minimal motor skills for activities such as moving objects, operating a telephone, personal computer, and/or most other office equipment including word processing, filing, and sorting of papers.
Visual demands require constantly reading documents for general understanding and analytical purposes.
