Job Summary

We are seeking a highly motivated and experienced Case Manager to join our team at the City of Somerville. The successful candidate will provide comprehensive case management services to residents in need of support, connecting them with resources and services to enhance their mental, social, and physical wellbeing.

Key Responsibilities
  • Conduct thorough assessments and develop personalized plans to address client concerns.
  • Establish and maintain relationships with community-based organizations and government agencies to identify and provide access to relevant resources and services.
  • Collaborate with City staff and other departments to coordinate services and ensure seamless referrals.
  • Provide education and support to clients on navigating forms and systems to access programs and services.
  • Maintain accurate and up-to-date client databases and produce required reports.
  • Stay informed about social service resources offered by the City, State, and non-profit agencies.
  • Develop and maintain strong partnerships with community organizations and service providers.
  • Conduct workshops and training sessions for internal City staff and external partner agencies on case management and oversight.
Requirements
  • Bachelor's Degree and 3-5 years of experience in case management services, or equivalent combination of education, training, and experience.
  • Licensed Social Workers (LSW) are strongly encouraged to apply.
  • CPR/First Aid Certification within three months of hire and ongoing maintenance.
  • Ability to travel independently to multiple locations within the City.
