Promise is recognized as a Federally Qualified Health Center (FQHC), committed to offering affordable and accessible high-quality health care services to the medically underserved populations. **This role will closely collaborate with our Substance Use Disorder program.**

The position involves delivering counseling services to children, adolescents, and adults, whether individually, in groups, or with families. Responsibilities include interviewing patients and their families to develop and implement programs that address their social and emotional needs, while working alongside primary medical, psychiatric, and oral health care providers to ensure integrated care.

Key Responsibilities:

  • Evaluate and assess the mental health needs of patients through referrals.
  • Provide timely direct counseling services to patients.
  • Draft therapy plans and measure patient outcomes.
  • Maintain comprehensive electronic health records related to counseling and patient interactions, ensuring compliance with funding requirements.
  • Make referrals for specialized services and monitor patient progress.
  • Collaborate with medical and social service providers to address patients' emotional and psychological needs.
  • Advocate for enhanced services for individuals with mental health challenges.
  • Assist in designing and implementing an effective mental health program.
  • Manage scheduling to ensure patient access to appointments.
  • Engage in ongoing education and maintain licensing requirements.
  • Perform additional duties as assigned.

Qualifications:

  • Exceptional verbal and written communication skills.
  • Familiarity with local medical and social service systems.
  • Strong documentation abilities and proficiency in electronic health records.
  • Ability to work both independently and collaboratively in a clinical environment.
  • Clinical skills to facilitate individual and group sessions.
  • Valid driver's license, insurance, and reliable transportation.
  • Willingness to work competently across various therapeutic modalities.
  • Strong interest in serving diverse and disadvantaged populations.
  • Ability to maintain composure in high-contact situations and utilize effective problem-solving strategies.
  • Commitment to professional ethics and confidentiality.

The candidate must hold a Master's Degree and possess a valid license as a Social Worker, Professional Counselor, or Marriage and Family Therapist. Preference will be given to Licensed Clinical Social Workers or Licensed Clinical Professional Counselors.
