Position Overview:
The Senior Care Support Specialist
  • Day Center assists participants in their residences or at the day center.
The Senior Care Support Specialist possesses the expertise and competencies necessary to deliver personal care and support for Activities of Daily Living (ADL) and Instrumental Activities of Daily Living (IADL) to elderly individuals who require assistance to maintain their independence within the community.

This role involves providing care and services as outlined in the participant's care plan, adhering to established regulations.

The hourly compensation for the Senior Care Support Specialist
  • Day Center at Center for Elders' Independence ranges from $21.42 to $32.13 per hour.
Compensation is determined based on market standards for the Senior Care Support Specialist position, as well as individual experience, skills, and work history.

Key Responsibilities:
Deliver Activities of Daily Living (ADL) and Independent Activities of Daily Living (IADL) support to participants, including: toileting, bathing, grooming, dressing, feeding, meal preparation, laundry, transfers, ambulation, and range of motion exercises.
Ensure safe transfers using Durable Medical Equipment (DME) as necessary, including lifts, gait belts, and hoyers.
Maintain cleanliness in participant areas, including bathroom sanitation and commode maintenance.
Provide appointment reminders to participants.
Keep the center or home orderly, including kitchen and activity spaces, while sanitizing relevant areas and equipment.
Assist participants to CEI transportation as needed.
Accurately document services provided in electronic health records in a timely manner.
Act as an advocate for participants by reporting any violations of their rights and addressing complaints promptly.
Report any changes in participant conditions to the supervisor or designated personnel.
Demonstrate reliability by consistently arriving on time for scheduled shifts and submitting accurate timesheets.
Maintain a professional, courteous demeanor while being responsive to all stakeholders, including team members, participants, and their families.
Adhere to all agency training requirements.

Home Care Qualifications:
A valid California Certified Nursing Assistant license or Certified Home Health Aide is required.
A High School Diploma or equivalent is preferred.
A valid California driver's license, current vehicle insurance, and access to a reliable vehicle are necessary for home care duties.
Current Basic Life Support CPR and First Aid certifications are required.
Proficiency in oral and written English is essential.
Basic computer skills are required.
A minimum of one year of experience working with frail, low-income elderly individuals is preferred.
Ability to work independently and collaboratively within a multidisciplinary team is required.
Bilingual skills are preferred but not mandatory.

Physical Requirements:

The physical demands outlined here are representative of those that must be met by an employee to successfully perform the essential functions of this role.

While fulfilling the responsibilities of this position, the employee is regularly required to stand, communicate, and demonstrate manual dexterity.

This role is physically active and requires frequent bending, kneeling, crouching, and reaching.

The employee must regularly lift, carry, or move up to 50 pounds and push or pull objects exceeding 150 pounds, using appropriate body mechanics.

Specific vision abilities required include close vision, distance vision, peripheral vision, color vision, depth perception, and the ability to adjust focus.

Work Environment:
In performing the duties of this role, the employee regularly encounters a diverse range of health conditions and environmental factors inherent to providing care for vulnerable seniors in various settings.
