Position Overview
The Outpatient Registered Nurse (RN) CAP 3 is a key role within the Clinical Advancement Program (CAP), achieved through progression from RN CAP 2. This position is responsible for delivering and coordinating high-quality clinical care, which includes assessment, planning, intervention, and evaluation for a designated patient group. The RN may delegate appropriate tasks to direct patient care staff, including RNs, LVN/LPNs, and Patient Care Technicians. As a vital member of the End Stage Kidney Disease (ESKD) healthcare team, this role involves participating in decision-making, educational initiatives, leadership activities, and quality improvement efforts aimed at enhancing patient care outcomes and operational efficiency.

Key Responsibilities
All tasks and responsibilities must be executed in alignment with company policies, nursing standards, and applicable regulations.


Under the guidance of the Supervisor and with support from the Clinical Educator or Preceptor, the RN will:


- Conduct systematic and ongoing collection and analysis of patient data before, during, and after hemodialysis treatment for assigned patients, documenting findings in the medical record, and making necessary adjustments to treatment plans while notifying the Team Leader, Charge Nurse, Supervisor, or Physician as required.


- Assess and document the learning needs of patients and families to provide initial and ongoing education.


- Collaborate with direct and ancillary patient care staff to ensure the safe and effective delivery of hemodialysis therapy.


- Administer medications as prescribed and document the rationale and effectiveness.


- Initiate or assist with emergency response protocols.


- Serve as a resource, leader, mentor, and role model for new and existing staff, exemplifying appropriate behavior and work ethics.


- Ensure proper laboratory collection, processing, and shipping procedures are followed, rescheduling any missed or inadequate collections.


- Identify expected outcomes and update nursing assessments and care plans in collaboration with the Interdisciplinary Team.


- Educate patients about transplant and treatment modality options.


- Participate in educational or quality improvement projects as directed by the Supervisor.


- May serve as a Preceptor for new employees.

Physical Requirements and Work Environment
This role involves direct patient care, which may require heavy lifting, moving patients, and assisting with ambulation. Equipment aids and/or coworkers may assist as needed.

- Frequent prolonged periods of standing are required, along with the ability to bend over.

- Occasionally, the employee may need to move machines and equipment weighing up to 200 lbs and lift chemical and water solutions weighing up to 30 lbs.

- The work environment is typical of a healthcare facility, featuring controlled air temperature and moderate noise levels.

- Exposure to infectious and contagious diseases/materials may occur.

- Daily tasks include desk work, computer use, and interaction with patients and healthcare staff.

- Travel to training sites or other facilities may be necessary.


May be required to perform essential functions in various locations with similar physical demands and working conditions.


Supervision
Oversight of Patient Care Technicians/LPNs/LVNs/RNs may be assigned as a Team Leader or designated Nurse in Charge after fulfilling the following requirements:
- Minimum of 3 months of experience in chronic/acute hemodialysis as an RN.

Qualifications
Education and Licensure:
- Graduate of an accredited School of Nursing.

- Current state licensure.

- Current or successful completion of CPR/BLS Certification.

- Must meet practice requirements in the state of employment.

Experience and Skills Required:
- At least 5 years of experience in Nephrology Nursing as an RN.

- Preferred experience in chronic/acute hemodialysis.
