Position Overview

We are seeking a dedicated Registered Nurse to join the outpatient practice within the Center for Advanced Cardiac Care (CACC), focusing on Advanced Heart Failure, Pulmonary Arterial Hypertension, LVAD, and Cardiac Transplantation programs. This role operates under the guidance of the Chief Nurse Practitioner and the direction of the CACC Medical Directors, ensuring high-quality patient care.

Key Responsibilities

  • Deliver comprehensive patient care coordination and education for patients and caregivers.
  • Support the ongoing clinical operations of the CACC advanced heart failure, LVAD, and transplant program, ensuring compassionate, patient-centered care.
  • Provide outpatient services, including symptom follow-up, triage, coordination of diagnostic testing, and review of diagnostic reports.
  • Actively participate in the daily management of patients and contribute to the vision of the CACC program.
  • Implement nursing care in collaboration with Nurse Practitioners (NPs) and Medical Doctors (MDs) within the Center for Advanced Cardiac Care.
  • Manage patient care in a clinical-administrative office setting.
  • Care for ambulatory patients with advanced heart failure, pulmonary arterial hypertension, LVAD, and pre- and post-heart transplant needs throughout their disease continuum.
  • Obtain and analyze laboratory and diagnostic test results.
  • Evaluate symptomatic patient inquiries and conduct telephonic triage.
  • Provide initial and ongoing education for patients and caregivers regarding medication, diet, and lifestyle management.
  • Administer nursing care in the clinic as directed by collaborating Physicians and Nurse Practitioners.
  • Complete clinical documentation in the outpatient medical record.
  • Conduct remote monitoring and assessments as prescribed by collaborating Physicians and Nurse Practitioners.
  • Utilize excellent communication skills and maintain flexibility in a dynamic care environment for complex patient management.
  • Adhere to all applicable HIPAA, billing compliance, and safety guidelines.
  • Perform additional duties as assigned that align with scope, education, and training.

Minimum Qualifications

  • Bachelor's Degree in Nursing (BSN) or equivalent education and experience, along with four years of nursing experience.
  • Current New York State RN license.
  • BLS certification.

Preferred Qualifications

  • Experience in telemetry, intensive care unit, coronary care unit, step-down unit, cardiology, cardiothoracic surgery, or emergency department.
  • ACLS certification is preferred.
