Position Overview

We are seeking a dedicated Registered Nurse to join the outpatient practice within the Center for Advanced Cardiac Care (CACC), focusing on Advanced Heart Failure, Pulmonary Arterial Hypertension, LVAD, and Cardiac Transplantation programs. This role operates under the guidance of the Chief Nurse Practitioner and in collaboration with the CACC Medical Directors. The Clinical Coordinator will work closely with the Chief Nurse Practitioner, ensuring high-quality patient care.

Key Responsibilities

  • Deliver comprehensive patient care coordination and education for patients and caregivers.
  • Support the ongoing clinical operations of the CACC advanced heart failure, LVAD, and transplant programs, ensuring compassionate, patient-centered care.
  • Provide outpatient services, including symptom follow-up, triage, coordination of diagnostic testing, and review of diagnostic reports.
  • Actively participate in the daily management of patients and contribute to the vision of the CACC program.
  • Implement nursing care in collaboration with Nurse Practitioners (NPs) and Medical Doctors (MDs) within the Center for Advanced Cardiac Care.
  • Engage in clinical-administrative tasks for effective patient care management.
  • Care for ambulatory patients experiencing advanced heart failure, pulmonary arterial hypertension, LVAD, and pre- and post-heart transplant, addressing their needs throughout the continuum of care.
  • Review laboratory and diagnostic test results to inform patient care.
  • Evaluate symptomatic patient inquiries and conduct telephonic triage as necessary.
  • Provide initial and ongoing education to patients and caregivers regarding medication, diet, and lifestyle management.
  • Administer nursing care in the clinic as directed by collaborating Physicians and Nurse Practitioners.
  • Ensure accurate clinical documentation in the outpatient medical record.
  • Conduct remote monitoring and assessments as prescribed by collaborating healthcare providers.
  • Utilize strong communication skills and maintain flexibility in a dynamic care environment.
  • Adhere to all applicable HIPAA regulations, billing compliance, and safety protocols.
  • Perform additional duties as assigned, appropriate to scope, education, and training.

Minimum Qualifications

  • Bachelor's Degree in Nursing (BSN) or equivalent education and experience, along with four years of nursing experience.
  • Current NYS RN license.
  • BLS certification.

Preferred Qualifications

  • Experience in telemetry, intensive care unit, coronary care unit, step-down unit, cardiology, cardiothoracic surgery, or emergency department.
  • ACLS certification is preferred.
