Advanced Cardiac Care Clinical Coordinator

Position Overview
We are seeking a dedicated Registered Nurse to join the outpatient practice within the Center for Advanced Cardiac Care (CACC), focusing on Advanced Heart Failure, Pulmonary Arterial Hypertension, LVAD, and Cardiac Transplantation programs. This role operates under the guidance of the Chief Nurse Practitioner and the CACC Medical Directors, ensuring high-quality patient care.
Key Responsibilities
- Deliver comprehensive patient care coordination and education for patients and their caregivers.
- Support the ongoing clinical operations of the CACC advanced heart failure, LVAD, and transplant program, emphasizing compassionate, patient-centered care.
- Provide outpatient services, including symptom monitoring, triage, coordination of diagnostic tests, and follow-up reviews of diagnostic reports.
- Actively participate in the daily management of patients, contributing to the vision of the CACC program.
- Implement nursing care in collaboration with Nurse Practitioners (NPs) and Medical Doctors (MDs) within the Center for Advanced Cardiac Care.
- Manage patient care in a clinical-administrative office environment.
- Care for ambulatory patients experiencing advanced heart failure, pulmonary arterial hypertension, LVAD, and pre- and post-heart transplant needs.
- Review laboratory and diagnostic test results to inform patient care.
- Evaluate symptomatic patient inquiries and conduct telephonic triage.
- Provide initial and ongoing education for patients and caregivers regarding medication, diet, and lifestyle management.
- Administer nursing care in the clinic as directed by collaborating Physicians and Nurse Practitioners.
- Maintain accurate clinical documentation in the outpatient medical record.
- Conduct remote monitoring and assessments as prescribed by collaborating healthcare providers.
- Utilize strong communication skills and adapt to a dynamic care environment for complex patient management.
- Adhere to all applicable HIPAA regulations, billing compliance, and safety guidelines.
- Perform additional duties as assigned, appropriate to scope, education, and training.
Minimum Qualifications
- Bachelor's Degree in Nursing (BSN) or equivalent education and experience, with a minimum of four years of nursing experience.
- Current New York State RN license.
- BLS certification.
Preferred Qualifications
- Experience in telemetry, intensive care unit, coronary care unit, step-down unit, cardiology, cardiothoracic surgery, or emergency department settings is preferred.
- ACLS certification is preferred.
